# EHI export

> How to export a patient's electronic health information, and the format of the exported file.

Last updated: 2026-10-29

Pharos exports a patient's electronic health information (EHI) as FHIR JSON. This page explains how to get an export and what the file contains.

## Single patient export

Tenant admins can download a patient's record as FHIR JSON from the patient's case in Spotlight. You can export a record whenever you need it, without contacting Pharos.

## Population export

To export the records of all your patients, email [support@pharos.health](mailto:support@pharos.health). Pharos runs the export for you and produces one file per case. Each file has the same format as a single patient export.

## Format

Each file is one [FHIR R4](https://hl7.org/fhir/R4/) [Bundle](https://hl7.org/fhir/R4/bundle.html) of type `collection`, encoded as JSON (`application/fhir+json`).

* The Bundle's `link` has an entry with relation `describedby` that points to this page.
* Each `entry` holds one resource. Resources are included unchanged, as Pharos stores them.
* For each extension the resources use, the Bundle includes a StructureDefinition that defines it.
* Every coding carries its `system` URI, so you can tell which code system each code comes from.

## Resources

An export can contain these FHIR R4 resource types:

* [AdverseEvent](https://hl7.org/fhir/R4/adverseevent.html)
* [AllergyIntolerance](https://hl7.org/fhir/R4/allergyintolerance.html)
* [Communication](https://hl7.org/fhir/R4/communication.html)
* [Condition](https://hl7.org/fhir/R4/condition.html)
* [Coverage](https://hl7.org/fhir/R4/coverage.html)
* [DeviceRequest](https://hl7.org/fhir/R4/devicerequest.html)
* [Encounter](https://hl7.org/fhir/R4/encounter.html)
* [Immunization](https://hl7.org/fhir/R4/immunization.html)
* [Location](https://hl7.org/fhir/R4/location.html)
* [MedicationAdministration](https://hl7.org/fhir/R4/medicationadministration.html)
* [MedicationDispense](https://hl7.org/fhir/R4/medicationdispense.html)
* [MedicationRequest](https://hl7.org/fhir/R4/medicationrequest.html)
* [Observation](https://hl7.org/fhir/R4/observation.html)
* [Organization](https://hl7.org/fhir/R4/organization.html)
* [Patient](https://hl7.org/fhir/R4/patient.html)
* [Practitioner](https://hl7.org/fhir/R4/practitioner.html)
* [PractitionerRole](https://hl7.org/fhir/R4/practitionerrole.html)
* [Procedure](https://hl7.org/fhir/R4/procedure.html)
* [ServiceRequest](https://hl7.org/fhir/R4/servicerequest.html)
* [StructureDefinition](https://hl7.org/fhir/R4/structuredefinition.html)

Other FHIR R4 resource types may appear. Use each resource's `resourceType` to identify it, rather than relying on this list.

The export doesn't include Pharos's own output: abstraction questions and answers, calculated measure results, reviews and workflow data. That is quality-measure and workflow data, not part of the patient's EHI.
